How do I submit the Form 5 paperwork for processing?

Simply FAX (please do not email) your paperwork to (502) 254-7016
When faxing correspondence, ALWAYS include your CAPID Number.
ALWAYS include a current Form 11 as a fax cover sheet.

 

I completed an FAA Flight Review in conjunction with my CAP Form 5. Why do I show up as only completing my previous Flight Review.

         Appropriate BFR/FR documentation was not received to update your flight
         
review.

What documentation do you need for a Flight Review, especially if you are doing one in conjunction with a Form 5?

         The applicant marking it in the Identification Block on the FRONT of the Form 5 does NOT constitute a BFR/FR.

         It ONLY lets us know to look for other documentation for an updated FR date.

         You MAY date the BACK of the Form 5, in the BFR section above where the applicant signs, to match the date of the Form 5.

         We MUST have a logbook copy with a SIGNATURE, a completed and SIGNED
         Form 11, or it marked appropriately on the BACK of the Form 5,  with the
 applicant's SIGNATURE.

         The trend here is SIGNATURE.

         IF an FAA Wings program is being completed and used in lieu of a BFR, we need    a copy of the FAA issued phase certificate, 
         
NOT the Wings card.

Do I need to Continue using the Form 12?

NO. The KYWG Form 12 is no longer used.

It has been replaced by the CURRENT version of the KYWG Form 11.
PLEASE include a signed copy along with any Form 5 paperwork.

 

Can ANY Flight release Officer release me for a Form 5, or Form 91 check ride?

NO
 

Who can release a Form 5, or a Form 91 check ride, in KWYG?

ONLY individuals indicated on the current Flight Release Officer (FRO) Authorization may release CAP check rides.

 

If I complete the Cadet Orientation Pilot portion of my Form 5, am I now a Cadet Orientation Ride Pilot?

No, you must complete the KYWG Form 11, have it signed and submitted along with your Form 5 paperwork. Once processed, you will be added to the Cadet Orientation pilot Authorization, and or MIMS. ONLY, at this time are you authorized to fly cadets in the Cadet O-Ride Program.
 

Why don't I show up on the KYWG Pilot Roster, MIMS, or other expected authorization?

More than likely, there is a piece of missing paperwork, or a piece of information that has not been received. You may want to follow up, by contacting the KYWG/DOV at dov@kywgcap.org
 

Can ANY Flight release Officer release me for a For Cadet Orientation Flight or ROTC Orientation Flight?

 

ONLY individuals indicated on the current Flight Release Officer (FRO) Authorization may release these flights.
 

Do I need to take the cadet orientation quiz every year?

No.
 

Common Form 5/Form 91 paperwork problems, or omissions.

No Date of Check on the FRONT top right of the Form 5/Form 91.
No Operations Officer signature on the BACK of the Form 5/Form 91.
No Aircraft questionnaires, or not identified and graded.
Paperwork is illegible.
No aircraft are listed in the comments portion of the Form 5.

Do I need to check the Night Orientation block, on the front of the Form 5?

NO, it need only be checked for a night proficiency check, if required by the wing commander.
 

How long after I complete the 60-1 Exam do I have to complete an Annual Form 5?

You MUST complete the CAPR 60-1 Exam within 90 days of completing an Annual Form 5.

 

If I am not doing well on my Form 5, or Form 91, may I discontinue the check ride and change it to a training activity?

NO. Once a checkride begins, usually at the oral, a checkride must be Satisfactory, Unsatisfactory, or Discontinued ONLY for weather, or maintenance problems, etc.
A discontinued check ride must be completed as Satisfactory or Unsatisfactory with the same check pilot, unless otherwise authorized be the KYWG/DOV.
 

May I complete a Form 91 while on a CAP mission?

A Form 91 MAY be completed at a training event, or SAREX.
CAPF 5 and/or CAPF 91 flight checks will NOT be conducted as part of an actual SAR/DR mission.
