04M206 - AFRCC -- IC Capt Fosbger - 4 Feb 04 - KyEM called the IC to ask about status of an ELT in Frankfort area.  IC called AFRCC and received mission.  GT was dispatched from LEX.  ELT was located in helicopter in Frankfort area.

04M0103 -AFRCC -- IC Maj Larry Herzog - 18 Jan 04 - AFRCC opened the incident at 0017Z for an ELT vicinty of Ft Knox and the Ohio River.  Ground Team from Ft Knox was dispatched.  ELT was located in an Illinois National Guard helicopter.  Incident closed at 0500Z.

 

03M2461 - AFRCC - IC LtCol Tom Schmitt - 21 Oct 03 - AFRCC opended the incident at 0000Z for an ELT vicinity of Campbellsville Airport (Taylor Co).  Aircraft was launched.  At 0013Z AFRCC reported another ELT vicinity of Grayson Co Airport.  Additional aircraft launched. AFRCC later reported both signals had gone silent and aircraft also reported no signal.  IC contacted KSP to ensure that Grayson Co ELT signal was not coming from aircraft involved in 03M2428.  That ELT was not broadcasting.

03M2428 - AFRCC - IC Maj Larry Herzog - 17 Oct 03-19 Oct 03  - AFRCC opened the incident at 1445Z for an ELT and overdue aircraft.  ELT was west of FTK and the overdue aircraft left Vine Grove enroute to OWB.  Pilot and passenger had not returned and aircraft was reported overdue by a relative on 17 Oct.  Ft Knox launched an UH-60 to assist with searching the reservation.  2 aircrafct were launched and a ground team assembled.  On 18 Oct 5 aircraft were launched.  Incoming leads led to new information on an ELT signal and a aircrew was diverted to DF the signal. Aircraft DFed signal to Nolan Lake.  KSP sent personnel and a helicopter to assist.  Grayson Co EMA also responded to assist in searching the lake.  By 2105Z, personnel on the ground identified the aircraft.  At this point CAP suspended operations and turned over incident command.

 

03M2017 - AFRCC - IC Maj Larry Herzog - AFRCC called at 1949Z with an ELT in the EVV, IN area.  IC called airport manager who did not hear signal but volunteered to drive to a nearby field where he located ELT.

03M1792 - AFRCC - IC LtCol Tom Schmitt - AFRCC opened above mission 11:45Z.  ELT, that had gone silent last evening was active again.  SARSAT not picking it up, however, over flying "heavies" were.  Launched aircraft which located ELT in Beech Travelaire.  Silenced approx. 14:55.

03M1783 - AFRCC - IC LtCol Tom Schmitt - AFRCC opened ELT mission at 18:00Z 6 Aug 03.  Signal in vicinity of Clark County Airport (IN).  Airport personnel located and deactivated.

03M1475A - AFRCC - Assistance to KyDEM - IC LtCol Henry Heaberlin - 8 Jul 03-9 Jul 03 - AFRCC opened the incident to support KyEM for a search of two missing children.  An aircraft was provided to conduct aerial search of the area on

 

03M1234 - AFRCC - IC Capt Kevin Fosberg - 15 Jun 03-22 Jun 03 - Multiple CAP wings, KyEM (and subordinate levels), and TEMA (and subordinate levels) involved in a search for a missing aircraft in Western KY.  Aircraft was located by local authorities in TN at a later date.

NHQ03NWS-12 - CAP NOC - Assistance to NWS - IC Capt Kevin Fosberg - 12 May 2003 - CAP National Operations Center again requested KyCAP to assist National Weather Service for an aerial photo and damage assessment flight.  This time the request was to take two NWS observers around Maysville.  High winds caused the flight to be postponed until 13 May 03.  During the 13 May flight, NWS personnel observed and photographed the path of a recent tornado near Maysville KY. National Weather Service contacts commented that they were pleased with the professionalism of CAP.  Additional information can be found on WMIRS.

 

NHQ03NWS-09 - CAP NOC - Assistance to NWS - IC Capt Galen Hall - 7 May 2003.  CAP National Operations Center (NOC) opened the incident at 1345z.  National Weather Service requested an aerial photo run over Paducah area to survey tornado damage.  NWS furnished the photographer who met the aircrew at Barkley Field in Paducah.  Aircraft was launched from Madisonville.  Additional information can be found on WMIRS.

03M0831A - AFRCC - Assistance to KyDEM - AL LtCol Tom Schmitt - 3 May 03 - AFRCC opened the incident for KyCAP at 2018z.  KyCAP was requested to assist by the state EOC to the Muhlenberg County SAR Coordinator.  The request was made for aircraft, ground team, and communications support.   Upon making contact with the SAR Coordinator, the AL was informed that the missing person had been seen in town and the SAR Coordinator was not sure if KyCAP resources would be needed.  An aircraft was put on standby in LOU and GTs from Bowling Green Composite Sq and Paducah Composite Sq were also put on standby.  The SAR Coordinator later contacted the AL again and informed that the subject was known to be in town and not missing in the woods and KyCAP resources were not needed.  The AL stood down all units on standby and closed the incident with AFRCC at 2320z.

 

03M0711 - AFRCC - IC Capt Kevin Fosberg - 18 Apr 03 - AFRCC activated for an ELT in Air Evac Lifeteam helicopter on a private field in Albany, KY.  Additional information can be found on WMIRS.

03M0674 - AFRCC - IC LtCol Jim Barnes - 14 Apr 03 - AFRCC opened the incident 0132z for ELT near Paducah.  SARSAT gave new merge and ELT located Mayfield Airport by GT from KY011, KY057, & KY217 in Cherokee 140.  Incident closed 0552z.

 

03M0564 - AFRCC - IC LtCol Tom Schmitt - 29 Mar 03 - AFRCC opened the incident 1703z for ELT near Grayson County (Litchfield) Airport.  GT on training near Otter Creek Brandenburg responded.  Just before GT arrived, airport personnel located and deactivated ELT in C172 that had been parked for over one year.  GT returned to campsite at 2120z and incident closed 2124z.

03M0433 - AFRCC - IC LtCol Tom Schmitt - 14 Mar 03 - AFRCC opened the incident 0155z for ELT near CVG Airport in NKY.  GT determined that ELT was trasmitting from LUK Airport in S.OH.  Due to the early A.M. hour, the incident was suspended until daytime and personnel could be contacted for entry to the hangers.  ELT was located and the incident closed at 1747z.

03M0284 - AFRCC - IC LtCol Tom Schmitt - 18 Feb 03 - AFRCC opened the incident early AM for an ELT in Northern KY.  The ELT was located 0800z by a GT.  The ELT was in an already crashed aircraft.  The antenna wire had been cut but the device was still transmitting.  Incident closed 0824z.

 

03M0270 - AFRCC - IC Capt Larry Herzog (14 Feb) & LtCol Tom Schmitt (15 Feb) - 14-15 Feb 03 - AFRCC opened the incident at 2138z on 14 Feb for an ELT near Buckner, KY.  Ground Teams from LCS and Falcon (IN) performed ramp checks at LOU and JVY.  Next series of SARSAT passes went negative.  Signal was not detected at either airport.  Incident was closed at 0200z on 15 Feb.  At 0345z, AFRCC reopened the incident after ELT went active again.  LtCol Schmitt assumed IC duties.  He dispatched the Falcon (IN).  The ELT was located at the Clark County Airport in a Lear Jet.  The crew and inadvertently activated the ELT cockpit switch.  Incident closed at 1525z on 15 Feb.

03M0153 (KyDEM #20030282) - AFRCC - IC Capt Larry Herzog - 25 Jan 03 - AFRCC opened the incident at 1348z on 25 Jan 03 for an aircraft missing out of Georgetown (27K).  Pilot Joe Polsgrove, left in a home-built experimental, took off 27K at 9 Jan 03 heading to MKC in Kansas City, MO.  The aircraft/pilot was reported overdue/missing on 24 Jan 03 by a friend.  Search operations are also being conducted in MO, IL, and IN.  Personnel were stood down at dark.  On 26 Jan 03 weather interfered with searching.  The IC conducted interviews and gathered information from ATC, FSS, witnesses, etc.  An aircrew out of LUK conducted a brief hasty search in the evening but quickly ran out of daylight.  On 27 Jan 03 the IC dispatched 2 Ground Teams (058 & 073) and an aircrew out of LUK.  Search operations were conducted.  An additional aircrew out of LOU was dispatched in the afternoon. Due to weather and ground cover, active searching in KY's area will resume on 29 Jan if the weather is favorable.  KyCAP operated with the Harrison County EMA, Harrison County Sheriff's Office, and other county and local officials for the search.  On the 28th, the IC coordinated with the ICs from MO and IL and AFRCC to share information and leads.  On the 29th 3 Ground Teams from NKY, KYANG, and LEX conducted search operations in Cynthiana again.  Bad weather prevented aircraft from flying even though three aircrews were available.  On the 30th, weather prevented aircrews from operating in Cynthiana again, but weather finally cleared in S.IN to allow an aircrew to search area.  During the search, LOU APCON diverted our aircrew to search for a Beechcraft that had gone short on landing.  However, the pilot was located and reported no injury; he landed in a field.  Our aircrew was directed back to its route search.  The IC also coordinated with the detective leading the missing person search.  Remaining time was spent following up on leads.  The incident also has gained a large amount of media coverage which has provided additional information.  On the 31st, weather over KY was IFR which prevented aircrews from being launched.  The IC conducted additional interviews and coordination efforts. On 2 Feb, additional grid searches were conducted in area around 018.  Additionally a route search from 27K to FFT was also covered.  On 3 Feb, weather kept the aircraft grounded.  On 4 Feb weather again hampered activity.  On 5 Feb, KyCAP conducted a route search from JVY to Vincennes up to 10nm wide.  Also searched a probable route from 27K to LOU up to 10nm north of that route.  On 6 Feb, weather kept the aircraft grounded for safety concerns.  On 7 Feb the weather was not productive for searching.    On 8 Feb several areas were covered by aircraft.  Debris was sited in a quarry lake.  A GT out of Lexington responded and coordinated with the Paris Fire Dept who attempted to put a boat into the water.  The ice, however, was too thick and operations were suspended.  On 9 Feb weather was forecasted IFR and 4 inches (+/-) of snow hit the area.  On 10 Feb weather again interfered with search efforts.  MO wing did conduct operations and followed up on a few leads.  On 11 Feb there we no operations as the snow hampers the search effort.  On 14 Feb, crews returned to the quarry lake.  Boat crews from Paris Fire Dept. using underwater cameras.  AFRCC suspended the incident on 19 Feb 03 at 1810z.  Additional information can be found on WMIRS.

 

03M0140 - AFRCC - IC Capt Kevin Fosberg - 23-24 Jan 03 - AFRCC opened the incident at 0606z on 23 Jan 03 for a 406MHz ELT with no 121.5 MHz carrier.  AFRCC had to merges as of time of activation in the vicinity of Bluegrass Army Depot.  Aircrews and Ground Teams dispatched by the IC did not detect any ELT signal.  At 2331z, AFRCC reported three negative SARSAT passes.  All remaining personnel were to told to RTB.  AFRCC closed the mission at 0112z on 24 Jan 03.  Involved were KSP Post 7, Madison County EMA, personnel from Bluegrass Army Detail, one Ground Team out of Lexington, one aircrew out of Louisville, and support personnel at Wing HQ.

03M0032 - AFRCC - IC LtCol Tom Schmitt - 5-8 Jan 03 - AFRCC opened the incident at 1716z on 5 Jan 03 for an ELT signal near the Lexington VOR.  IC contacted Standiford Field Tower who reported they were not receiving the signal as were none of the heavies in the area.  Aircrew launched at 1840z and reported no signal.  AFRCC then reported that the signal had gone dead.  At 1930z, AFRCC reported that ELT had again went active.  Aircraft was at time refueling on ground.  Aircraft was again launched and did not receive signal.  Ground Team was dispatched to check location of coordinates of the 7 SARSAT hits.  No ELT signal detected.  Mission Suspended at 0422z on 6 Jan 03.  AFRCC resumed the mission on 7 Jan 03.  Aircrew launched at 1815z.  Picked up ELT signal at 1835z.  The target was located at Blue Lick Airport at 1845z in a Cessna 172.  Water had entered the ELT casing and wind shaking the aircraft caused the intermittent signal from the ELT.  ELT removed from aircraft by mechanic and deactivated.  ELT signal had been carrier-only (no audio).  AFRCC closed mission at 0005z on 8 Jan 03.

 

02M2896 - AFRCC- IC LtCol Henry Heaberlin - 16 Dec 2002 - AFRCC opened the incident at 11:43 pm EST (0443z) for an ELT signal tracked to Samuel's Field, Bardstown, KY.  IC contacted Bardstown Commander at 0502 to dispatch team to locate the ELT.  0530z, team located and deactivated ELT, in the team leader's aircraft.  All personnel home safe by 0700z and mission closed with AFRCC at 0709z.

02M2740 - AFRCC - IC LtCol Tom Schmitt - 20 Nov 2002 - AFRCC opened the incident at 0026z for a missing aircraft that had departed from Wilkes-Arre, PA enroute to Knoxville, TN.  The aircraft had departed at 1530z and was overdue.  The IC had all FSS conduct ramp checks.  A low flying aircraft had been sighted in Perry County, Whitely County, Bell County, and Harlan County.  A hospital in Pinneville reported seeing a low flying aircraft with smoke coming out of "one of its engines."  They reported the tail number and it did not match his aircraft.  The tail number of the aircraft belonged to a pilot in Bar Harbor, MN.  The pilot confirmed his aircraft was not missing.  At 0050z, Elkins FSS reported that the aircraft safely landed in Beckley, WV.  All states responding except KY were closed.  KY was kept open in case ELT was detected in the state.  KY was closed at 1120z with no reports of ELT or missing aircraft.

 

02M716A - AFRCC - IC LtCol Tom Schmitt - 17 Nov 2002 - A hunter entered the forest near Ft Knox, KY on 16 Nov 2002.  He remained in forest overnight and the request came from AFRCC on 17 Nov 02.  KY Wing mobilized approx 45 personnel to respond, including but not limited to, aircrew for transport, ground teams, mission base staff, and SAR K-9s handled by ground team personnel.  The Army had begun the search and the hunter heard K-9s and walked out on his own.  KY Wing stood down before deploying to the field.
