KY Wing Pilot and FRO Paperwork

To ALL KY Wing Pilots, Crew Members, and FRO's:

It's a constant challenge to keep pilots/crew informed well enough so all of our documents are completed on time, accurate, and consistent. We must each constantly strive for excellence in detail, accuracy, and consistency. Kentucky is one of the best wings in the Nation, if not the very best, and we want to live up to the reputation we've established. Here are a few items for all of us to keep in mind.


1. PIC's:

  • The PIC of every flight must first have received a flight release from an authorized FRO. Regulations require that the FRO only give a release to the PIC of the flight. Crew members can not call for a flight release.
  • The PIC during a Form-5/Form-91 check ride is, under most circumstances, the candidate taking the check ride. We treat our check rides same as FAA check rides and the candidate is the PIC even though he/she may not be technically qualified in the aircraft yet.
  • ALL wing forms related to a flight must be consistent between one document and another. That means when a FRO releases a flight to PIC John Doe, the FRO lists John Doe as PIC on his CAPF-99 flight release form, along with any crew members. John Doe MUST in turn be listed as the PIC on the Aircraft Tach Sheet. Tach Sheets should also LIST ALL crew members on each flight. ALL forms related to a flight must also reflect John Doe as PIC and list all crew members, such as form-94's, form-104's, form-107's, form-115's, Weight/Balance Sheets, and Crew Data Sheets.
  • All paperwork, including checks, is the responsibility of the PIC and must be mailed in immediately after each flight. Crew members may send in paperwork, but we will come looking for the PIC who is ultimately responsible for each flights paperwork, including accuracy, timeliness, and any payments due. After each flight, PIC's report Ending Tach, Ending Hobbs, and any Squawks to their FRO.

 

2. Check Rides:

  • AF approved and funded check rides are available for qualified pilots. ALL initial check rides must be flown as a self funded check rides and check pilots should make sure the applicant understands their ride is self funded.
  • Only certain FRO's in the KY Wing are authorized to release Form-5 and Form-91 check rides. Check the Wing Stan/Eval Web Site for the Authorized Flight Release Officers for Form-5's and Form-91's
  • The PIC should make sure that "Form-5" or "Form-91" is written in the "SQUAWKS/COMMENTS" column on the Tach Sheet regardless of the mission symbol. This simple notation is the only way we have of the keeping accurate track of all check rides given in our wing.
  • In addition to the normal report of Ending Tach, Ending Hobbs, and any Squawks to their FRO, the PIC should also give the FRO the fuel amount after each check ride.

 

3. B12 Flights:

  • All B-12 Proficiency flights may only be flown by qualified SAR/DR/CD mission pilots (not leading to an airman rating or certificate) or properly supervised trainees (with a Qualified Mission Pilot aboard). Complying with CAPR 60-3; this means meeting all the Prerequisites and familiarization and pre-training requirements BEFORE serving in that position on actual or training mission under supervision.
  • The transportation mission profile may only be flown by FAA commercial rated pilots or qualified SAR/DR mission pilots.

 

4. FRO's:

  • Are required to enter ALL releases into WMIRS and close them out within 72 hours.
  • Are required to track B12 profiles numbers on their capf-99's.
  • Are required to e-mail a report after each each flight directly from their capf-99.
  • Are required to e-mail a copy of their capf-99 file to the current operations officer NLT the 2nd of each month..
  • Are required to print/sign/mail their capf-99 at the end of each month to the current operations officer.
